落實健康人權是公共衛生的使命，但面對健康不平等的艱鉅挑戰，我們可以從過去學到哪些教訓本文目的不在於完整回顧人類追求全民健康的歷史，而是分享三個歷史教訓第一，消除健康不平等是政治議題第二，社會條件是造成健康不平等的根本原因以及第三，政治及社會條件本是公共衛生的要素。 Mahler主張的社會革命有兩個基本要素人人健康 (health for all) 與初級醫療保健服務 (primary health care)。他說人人健康是社會價值體系，而初級醫療保健服務則是達成人人健康的策略又說這兩個基本要素不能分開，因為我們必須清楚價值會帶我們去哪裡，以及到那裡要靠初級醫療保健服務。 2005年，世界衛生組織成立健康之社會決定因素委員會 (The Commission on Social Determinants of Health)，李鍾郁任命Michael Marmot爵士為主席，並且在成立大會致詞. 1920年，美國耶魯大學Charles E.A.
