Despite the seemingly endless train of decisions that have to be made, there's something really nice about sitting on your porch and looking out on your neighborhood. As 4:30pm rolls around on my first call nigh, the fear is intense and palpable, my scalp starts to itch, my heart starts to race, and I briefly imagine simply turning off my pager and curling up into a ball in the call room. Thursday, June 19, 2008. Alert and Oriented X 3. There's an anxiety in the unknown that lies ahead in the next few weeks and months. At least I'm more aware now that this is simply how you feel before starting something completely new (and a little scary). This is what the start of medical school and the start of third year felt like, except intensified. I feel much more aware (and wary? Of what's happening around me, because it feels like it "counts" more than it ever has before. Let me give you my card.

There were a number of occasions in which a patient's comfort or preference conflicted with the best interests of the patient, in terms of his/her health. On closer examination, however, it was clear that he was not just asleep. He was in the deep slumber of someone who is not merely resting, but more importantly, may not be able to wake up.
