Inflammations of the Breast

Usually occurs in the lactating breast. Pus drained on aspiration. Localised swelling /- bruising. Clinically may mimic cancer. Resolves spontaneously but slowly. FNA or core biopsy good for tissue diagnosis. May be associated with microcalcification - see also benign microcalcification (3). Chronic fat necrosis with oil cyst formation - left: cystic space formed by necrosis of adipose tissue and lined by foamy macrophages and foreign body giant cells (right). May present as a lump or nipple discharge.
Gynaecomastia

Common, often unilateral retroareolar swelling in MALES. May occur soon after puberty or alternatively in older males. Usually idiopathic but associations with some drugs (e.g. spironolactone, cimetidine and cannabis) and liver disease. Clinical diagnosis usually straightforward. Often similar to the range seen in fibroadenomas - may be worrying. Excised if clinically suspicious or causing discomfort/embarassment to patient. Mouse over' - for closer view - note mild epithelial hyperplasia in ducts.
