Core Concepts in EM; a repository for Carolinas lecture summaries. Sean Fox, MD. Peaked T-waves are narrow and symmetric. Wide and irregular QRS morphology. Strongly consider when QRS duration greater than 200 ms. Give calcium early on in the management course. Classically, deep symmetric T wave in V1-4. Less commonly, ST elevation with biphasic T wave in V2-3. Highly specific for proximal LAD occlusion. Subscribe to comments on this post ). May 17th, 2012 - 16:18. September 11th, 2013 - 05:32. Core Concepts in EM; a repository for Carolinas lecture summaries. Michael Koehler, MD. SBP is the most common bacterial infection in patients with cirrhosis. Suspect SBP in a patient with known ascites who presents with any abdominal discomfort, worsening ascites, encephalopathy, or ileus. Fever is present in only half of SBP cases. A diagnostic peritoneal tap is a quick and easy procedure with rare complications. First-line treatment for SBP is 2g of cefotaxime IV. Filed under: Core Concepts. Producent...

Core Concepts in EM; a repository for Carolinas lecture summaries. The Art of Calling a Consult. Matthew D. Sullivan, MD. Succinct Consultation is a required skill set for Emergency Physicians. In order to maximally improve your communications in consultation you must practice. Pitfalls in consultation include: consultation at night, cross-covering physicians, change of shift. Anticipating stumbling blocks in consultation is the best way to avoid consultative miscues. Matt Sullivan, M.D. August 16, 2016.
M&M – PE and Airway Management | Carolinas Core Concepts
http://www.carolinascoreconcepts.com/mm-pe-and-airway-management
Core Concepts in EM; a repository for Carolinas lecture summaries. M&M – PE and Airway Management. Kelley Cardeira, MD. 13% prevalence of thromboembolic disease in all COPD patients. 30% prevalence in atypical presentations. Those at high risk of contrast "allergy" are those with previous reactions, asthma, or ANY severe allergy (not more likely with seafood allergy). Pre-treatment only works if given at least 12 hours prior to contrast load. Use lots of nebulized and spray lidocaine for awake intubations.
Thyroid Disorders | Carolinas Core Concepts
http://www.carolinascoreconcepts.com/thyroid-disorders
Core Concepts in EM; a repository for Carolinas lecture summaries. Dave Ahlers, MD. Myxedema coma and thyroid storm are a clinical diagnosis, start treatment without testing if suspicious. For thyroid storm give beta blocker (propanolol or esmolol) then thionamide (methimazole) followed by iodide solution (SSKI or Lugols) 1 hour later. Myxedema coma patients need hormone replacement ASAP (ie: T4 /- T3). All thyroid disorder patients in crisis should get stress dose steroids. Filed under: Core Concepts.

Medicine | Agraphia

I came across this blog post. Recently by a retired surgical chair who is pretty upset with a court case about an emergency physician. People are demonizing the physician for forcing this poor man to undergo a "warrantless search" and being a "tool of law enforcement." What I find so frustrating is how quickly people decry the actions of this emergency doc - and assume that law enforcement was just out to make an unconstitutional bust. In my mind, this is a medical case, not a legal one. Your patient act...
drug seekers | Agraphia

I Can’t Get No) Satisfaction. My resident told me today that he was planning on giving antibiotics to a kid with a fever. I asked him where the fever was coming from - he said he didn't see anything on the exam, but the parents were pretty insistent that the kid needed antibiotics, and he was tired of arguing. "Plus," he added, "it's good for patient satisfaction scores.". But he's got a fever," she said, "every time he has a fever my pediatrician puts him on antibiotics. My child. If you think this does...

How To Fix The Prescription Drug Problem. As I am wont to do lately, I've been thinking an awful lot about prescription drug abuse. My last shift, I saw a guy who got in two car wrecks in the space of a few hours while taking his 2mg prescription Xanax "bars". Six times a day. I believe the solution lies with the state medical licensing board. Here's my plan. The state board sends all physicians an alert "Your license may be in jeopardy! The end goal is to have meds prescribed as intended, with short cou...
An Argument Against The Poppy Seed | Agraphia
http://www.agraphia.net/an-argument-against-the-poppy-see
An Argument Against The Poppy Seed. I know I talk a lot lately about drug seeking. A large part of this is that I work as the community ER doctor in a small town rife with opiate and benzodiazepine addiction. It's a massive issue. During one particularly miserable shift, I decided to catalog the degree of drug seeking behavior. I saw 25 patients. Of those, 15 had presented to the ER for overuse of opiates. These split into three categories:. Came in by EMS not breathing due to an opiate overdose. This br...

Ripping It Up | Agraphia
http://www.agraphia.net/ripping-it-up
One of my favorite things is when patients rip up a prescription - it's just such an unnecessary, childish action. Usually it goes something like this:. I need purpleset tens for my back pain. My doctor is out of town.". That's Percocet, and the fact that you abbreviate the dose 10/325 as 'tens' makes me concerned that you're far too familiar with this medication. We're going to give you some Tylenol today.". I can't take Tylenol.". You know there is Tylenol in Percocet, right? Tagged as: drug seekers.
