這次的處方是沙參麥冬湯 (去桑葉、玉竹和甘草) 作主方，加入參苓白朮散 (去大棗、淮山和蓮子)，再給三劑，6碗煮取2碗，分早晚服，並叮囑這個新年要食得清淡，不要太開懷。 他問 我已切除了扁桃腺，何以還有喉嚨痛 我笑說 你有喉嚨嘛，當然可以患喉嚨痛 他感到剛才所問的問題很笨，露出了尷尬的樣子，我續說 由11月以來，你就得了肺燥的病機，這使你的氣血一直鬱積在咽喉部位，所以出現了喉嚨發炎和扁桃腺炎。 人身五臟，不是血肉之五個器官，而是氣血 (能量) 運行的五種狀態，當中的肺金，主收歛肅降 (能量)，能歛能降，則人之上身就不會被氣血 (能量) 所鬱積，不會發生喉嚨痛。 上身氣血多，故流涕噴嚏 (氣血多得要泄出來，很浪費)，自覺發熱 (上身廣泛地方氣血多了) 下身氣血少，使氣血源頭枯竭，無以為繼，故疲倦怕冷，連腸胃也運作不佳，所以腹部隱痛，時時欲瀉。 初診的症狀和舌脈，明顯是風木泄動之象，故用桂枝湯 (有關桂枝湯的分析，見 外感病案一 http:/ www.cmedinclassics.hk/%E5%A4%96%E6%84%9F%E7%97%85%E6%A1%88%E4%B8%80/.
