查体 该患儿智力正常，言语清晰，查体合作，双上肢无明显异常，双股四头肌肌张力III级，肌力3级 右下肢小腿三头肌肌张力IV级，呈强直状态 左下肢小腿三头肌肌张力III级 右踝背伸肌力1级，跖屈肌力2级 左踝背伸肌力1 级，跖屈肌力2级 右侧踝关节背伸活动度受限 股内收肌角45 站立平衡2级 可扶走，双足呈马蹄内翻畸形，右侧为重，双足跟腱挛缩，下蹲时左足跟离地6cm，右足跟离地8cm。 治疗 因患儿双下肢畸形较重，且跟腱挛缩严重，双下肢肌张力较高，故由八一脑瘫康复中心孙成彦教授制定治疗方案 1.先行 跟腱延长术 及 内收肌部分切断术 为该脑瘫患儿打下康复的基础 2.进行长时间康复训练增加肌力以及改善关节活动度，并适当的降低肌张力 3.再进行脑瘫手术评估，行 功能性选择性脊神经后根部分切断术 ，降低双下肢肌张力 4.行 脑瘫肌力、肌张力调整术 ，改善脑瘫患儿双下肢畸形情况 5.最后再次进行康复治疗，重新建立正常人体的运动模式。 故该脑瘫患儿的治疗方案为 跟腱延长术及内收肌部分切断术 康复训练 功能性选择性脊神经后根部分切断术 脑瘫肌力、肌张力调整术 康复训练共五个阶段。
