Tuesday, March 27, 2012. Lung Point in M-mode. This week’s image of the week is brought to us by interns Deepa Patel, Dilani Weerasuriya, and soon to be new intern- Class of 2015, Lauren Hudak. They obtained an M-mode image of the "Lung Point", the only ultrasonographic finding that is highly specific for pneumothorax. As part of our E-FAST exam we typically look for lung sliding in the second intercostal space just lateral to the sternum. We use the presence of lung sliding to rule out pneumothorax.

Friday, February 24, 2012. Complex effusions can be more difficult to distinguish from consolidated lung. the video posted below shows an empyema secondary to TB. To distinguish the two you can look for air bronchograms (small bright reflectors from air trapped in the lung), B-lines artifacts at the edge of the lung, or doppler can be used to visualize small pulmonary vessels. These three helped evaluate a patient with Dr. Huot and Candace Gates, an off-service resident rotating in the ED.
