This isn't actually part of your dossier, but you will need to fill out this form near the end of your adoption and send it down to Haiti. I submitted mine as soon as I learned we were in MOI. Here's how to fill it out. (One per child, please! 1) Family Name: your last name, First Name: your child's first name, Middle Name: your child's Haitian last name. 2) Other Names: leave blank. 4) date of birth for child being adopted.

Tuesday, January 6, 2009. Chosen: where one becomes two. Despite their situations, their eyes showed no trace of saddness! How could i not be moved by what i saw. babies,toddlers,tweens oh my! Eyes wide and smiles big! Some in waiting for paperwork to process, some waiting to be chosen. They have to be chosen! I can't fathom it-. Here is where i choke up. Here is where my tears can not be contained. My little one, dieunika chose me. I did not seek her. I have no words adequate or elegant enough to explain.
