This is the welcome screen from which the user selects to either create a new database stored as dbname.ldb in $HOME/.openchange/ or select one that is already existing. This is the create new database option, where the user gets to enter a name of the database to create. This is what the user gets in case he/she selects the select an existing database option, one can then navigate to select the database of choice in the folder of choice.
