Request a PEERRS Human Subjects Certification Waiver. If you are requesting a PEERRS wavier on behalf of someone else, please use that person's information when completing the form. U-M uniqname or Friend Account. The ID provided MUST BE THE SAME as the ID used in eResearch. This is necessary for the waivers to be linked properly in eResearch.). Name of PI on IRB Submission.
