How to Begin Admission Process. Should you decide to make a formal application for services, an admissions coordinator will help you through the process. Before an interview is set up and a decision is made, the admissions team would like to have the following information:. Approved Certificate of Need form. Current Comprehensive Clinical Assessment recommending PRTF placement. Person-Centered Plan (PCP) with a doctor’s signature indicating PRTF services. Current CALOCUS with a score of 28 or higher.
