一) 傳真申請 衛生福利資料科學中心 以下簡稱本中心 網頁下載並填寫使用資料申請單後，傳真相關申請文件至衛生福利部統計處(以下簡稱本處)吳姿慧小姐。 申請使用本中心資料庫，需填具 使用資料申請單(APP001) 、 檔案欄位勾選表(CHK)。 註1 依 使用資料申請單 中申請使用之檔案，依序填列 檔案欄位勾選表。 註2 檔案欄位勾選表 中 資料欄位需求之緣由或目的 之欄位請務必填寫 經審查發現未填寫或目的不明確者，該欄位不予提供。 一) 收費方式 使用本中心資料依據 衛生福利統計資料使用收費標準 (LAW001) 收費 費用收取後，概不退費。 一) 資料處理 本中心於收到繳費確認通知後，方進行資料編輯下載處理作業 每一申請案以7 21個工作日提供為原則，若作業地點為研究分中心者，資料準備時間約14 28個工作日提供為原則。 一) 攜帶文件 分析人員首次前來本中心獨立作業區分析時，需攜帶並當場繳交申請者及所有使用人員之正本 保密切結書(SEC001) 、正本 使用聲明書(SEC002) 、 一個月內脫帽半身二吋照片一張 及 含照片之身分證明證件 (如 身份證、健保卡、駕照、護照等)，以製作識別證。
