現病歴：二年ぶりに喫煙を再開したところ，3日後から呼吸苦，咳嗽などが出現．発熱もあったため受診となった． 検査所見：WBC 11,600，CRP 16．9 ，SpO2 87％（RA）. 経過：経過からAEPが疑われたため，気管支肺胞洗浄が行われた．BALFで好酸球の増加を認め，確定診断となった．ステロイドによる治療が行われ，速やかな症状の改善がみられ，2週間後のフォローのCTでは病変は消失していた． AEPは20歳前後の健常な若年者に発症することが多い．原因は不明なものが多いが，喫煙，薬剤，真菌，金属吸入などの関与が報告されている．特に喫煙との関係を示唆する報告が多く，ほとんどの症例は喫煙開始後数週間以内に発症する． 胸部単純X 線写真では，両側肺野のびまん性陰影やKerley' s line が見られることが多い． 胸部単純CTの特徴的な画像所見はびまん性に分布する高吸収域，小葉間隔壁の肥厚，気管支血管束の肥厚， 胸水などが認められる． 現病歴：1か月前より左顎に疼痛出現．明らかなきっかけはない．顎運動にて疼痛が増. 診断：insulinoma（疑い）.
