Committed to providing client centered services. Outreach Case Management Program. Outreach Case Management Program. A flexible case management/support service is provided to people living in their own homes in Burlington, Acton, Milton and Georgetown. Intensity of service is based on need, for example, some clients may need two to three visits a week, while others may only require one visit a week. Others may only need one visit a month to enable them to maintain their healthy lifestyle.
